If you are requesting a full PTSD, Anxiety, or Opioid Use Disorder evaluation with no prior medical records, please indicate that on your appointment request form.

What next?

Upon using the scheduling link, you will be able to provide the information necessary for me to complete your cannabis application. After we have our appointment, I will upload your application to your NMDOH patient portal. NMDOH will then send you a request to sign in to your patient portal and complete your application. After completing the application, your cannabis card will be issued digitally (you may print a copy if a physical card is desired). Waiting times vary from 1 week to 2 months. Please be advised that payment for my service will be accepted via a secure payment link at scheduling and due the day of the appointment.
